TREMBLAY, Jean-Philippe - On June 12, 2007, accidentally passed away in Chicoutimi, at the age of 18 years and 8 months, Mr. Jean-Philippe Tremblay son of Mrs. Sylvie Paquette and Mr. Fabien Tremblay, residing at 323, rue de la Gaillarde Chicoutimi. The family will welcome relatives and friends on Friday, June 15, 2007 from 10:30 a.m. to 1:15 p.m. at the Alliance funéraire du Royaume Residence Ste-Claire 345, rue St-Armand, Chicoutimi The funeral will take place on Friday, June 15 June 2007 at 2 p.m. at Ste-Claire Church. He will be cremated at the Kingdom Funeral Alliance crematorium. He is survived by his parents: Dame Sylvie Paquette and Mr. Fabien Tremblay; his sisters: Carolane (Alex Gamache) and Kassandra; his lover Claudia Gobeil; his maternal grandparents: Gaétane Savard and Jean-Pierre Paquette (Cécile Santerre); his paternal grandparents: Candide Tremblay and Gérard Tremblay (Colette Pilote); his aunts and uncle: Manon Paquette (Serge Tremblay), Maude Paquette (Dany Lévesque) and Serge Paquette (Manon Vaillancourt); his cousins: Maxime Tremblay (Catherine Barrette-Dufour), Kaven Tremblay, Dave Tremblay (Mélissa Tremblay), Yannick Tremblay, Alex Tremblay, Alexandra Laberge, Louis Laberge, Guillaume Lévesque, Joanie Paquette and Alexandre Martin. He also leaves many friends. The family would like to especially thank Dr. Hans Mc Lelland, as well as all the staff of the pediatric department of the Chicoutimi hospital for the care provided to Jean-Philippe, as well as their support over the past few years. For people wishing to offer donations a volunteer from the Children's Wish Foundation will be present at the funeral home to collect them.
